There are two primary types of home health care services, though sometimes it can be hard to determine which is the right fit for you. Within Community First’s health care continuum, we offer both clinical and non-clinical support you can count on by one trusted provider.

Colonial at Home provides non-clinical support to help with everyday activities like bathing, dressing, meal preparation and household tasks to enable independence and safety. This type of home care can be customized to your loved one’s specific needs and does not require any qualifying medical conditions or a physician referral. This service is private pay. Rates are billed hourly, and there is a minimum visit of two hours.

Bridges Rehab at Home provides short-term clinical nursing care and therapy following an injury or illness. To be eligible for services, you must have a temporary homebound status and be referred by a physician. Home health care is generally reimbursed through Medicare or insurance plans.

Colonial at Home Packages

Companionship & Respite

Your caregiver will visit to provide companionship. From just playing cards to running errands, we will make sure your needs are met. We also offer temporary respite care.

Get Up & Go

Many clients need just a bit of help getting the day started. For 2-4 hours each morning, your caregiver will assist with everything from making the bed to cooking breakfast, personal grooming and scheduling plans. We offer similar services at the end of the day to assist at bedtime.

Health & Wellness Support

Caregivers can offer vital assistance to individuals managing chronic conditions or recovering from illness or surgery. We offer support with bathing and dressing, managing medication, transportation to doctor appointments, and all other client needs.

Assisted Living at Home

Individuals who need extra care, yet don’t want to leave the comfort of their home, may benefit from our full-time support. Caregivers are trained to provide dementia care, chronic condition management and support for major health changes. We are able to assist families in providing the best support to their loved ones from 2-24 hours per day.

Healthy at Home

Provides short-term assistance after coming home from the hospital or leaving rehab. Get up to 14 hours of assistance over a 10-day period.

About Bridges Rehab at Home

Following hospitalization or a stay at a skilled nursing facility, Bridges Rehab at Home provides specialized skilled care within 24 hours of your return home. We develop health care solutions specific to your unique needs and provide motivation, encouragement, and education along with our top-rated therapy and nursing care. We offer an array of specialized therapy programs and chronic condition support. Our team works with your physician to develop a customized program to support your unique health care needs.

How to Pay

  • Insurance: Home health can be covered by medical insurance, including Medicare and Medicaid.
  • Must be referred: Home health must be prescribed by a physician.
  • Eligibility: Clients must have a temporary or permanent “homebound” status.

Ortho Program

Orthopedic rehabilitation is a form of therapy that treats a large variety of conditions that affect the skeletal and muscular systems. Each patient receives a patient-specific and specialized rehabilitation plan. Our team of rehabilitation experts targets the specific needs of the patient and develops strategies to maximize each patient’s function, with up to five days a week (including weekends) of Speech, Occupational and Physical Therapy.

Cardiac Rehabilitation

Cardiac rehabilitation is a medically supervised program that helps improve the health and well-being of people who have heart problems. Rehab programs include exercise training, education on heart healthy living, and counseling to reduce stress and help you return to an active life.

Pulmonary Rehabilitation

Pulmonary rehabilitation is a program of education and exercise classes that teaches you about your lungs, how to exercise and do activities with less shortness of breath, and how to “live” better with your lung condition.

Bladder Health

The Bladder Health Program is supervised by a nurse practitioner specialized in geriatric bladder health and treatments, including assessment, therapies, exercises and training that supports healthy lifestyle changes.

Medication Management

The Medication Compliance and Education Program employs our unique Health Coach at Home goal orientation techniques to clarify and educate on medications. We teach when, why and how medications should be taken.

Memory Care

The Memory Care Program team uses a special blend of compassion, care and training to work with clients who face the challenges of cognitive and memory deficits. Our team of clinicians works with patients and their loved ones to build effective communication methods and formulate strategies and interventions for memory recall.

Swallowing

The Swallowing Program employs a dedicated team of speech language pathologists who are skilled at identifying swallowing issues and creating a therapeutic program to improve the ability to swallow safely.

Wound Care

The Wound Care Program utilizes state-of-the-art wound care technology which allows physicians to monitor their homebound patients’ wound status. Our Wound Care Consultant works with a skilled nursing team to provide the latest in wound healing techniques and care. Physicians can monitor wound progress by comparing weekly wound measurements as well as other vital reports regarding patients’ progress 24/7 via a HIPAA-compliant, secure website.

Diabetes Management

The Diabetes Management Program focuses on controlling diabetes through education, proactive diet, exercise and medication therapies. Our specialists assist patients in making long-term changes to improve their quality of life.

Heart Failure Management

The Heart Failure Management Program provides critical medical care while emphasizing education, medication management, diet and activity guidelines, and self-monitoring of the disease.

COPD (Chronic Obstructive Pulmonary Disease) Management

The COPD (Chronic Obstructive Pulmonary Disease) Management targets education, treatments and lifestyle changes that can help patients with chronic bronchitis, emphysema, chronic asthma, lung cancer, pneumonia, tuberculosis and other lung diseases.
